Diuretic Therapy for Extremely Preterm Infants Does Not Alleviate Respiratory Problems

News Release
Monday, March 26, 2018 Diuretic therapy — commonly given to extremely preterm infants to help them overcome respiratory problems — appears to offer no benefit for this purpose, according to an analysis by researchers at the National Institutes of Health. Surprisingly, infants in the study who received diuretic therapy were more likely to require respiratory support, compared to extremely preterm infants with similar respiratory problems who did not receive the therapy. The study is published in The Journal of Pediatrics.
Diuretic medicines prompt the kidneys to make more urine. The therapy is commonly given to preterm infants to help drain fluid from the lungs, but there is little research evidence to support the practice.
Researchers at NIH’s Eunice Kennedy Shriver National Institute of Child Health & Human Development (NICHD) & National Heart, Lung, & Blood Institute (NHLBI) analyzed data collected as part of the Prematurity & Respiratory Outcomes Program study, an effort to understand respiratory difficulties in extremely premature infants during the first year of life. The study included 835 infants born between 23 & 28 weeks at 13 U.S. neonatal intensive care units. An infant is considered full term at 39 weeks.
Researchers analyzed the infants’ daily medication and respiratory support records from birth through 34 weeks. They found that 90 percent of the infants on diuretics needed supplemental oxygen, compared to 59 percent of infants who did not receive the therapy. On the day prior to the first diuretic treatment (14 days of life), 56 percent of the diuretic group…

Atypical Brain Development Observed in Preschoolers with ADHD Symptoms

News Release
Monday, March 26, 2018 NIH-funded study uses high-resolution brain scans to uncover structural changes. Children as young as 4 years old with symptoms of attention deficit hyperactivity disorder (ADHD) may have significant differences in brain structure, compared to children without such symptoms, according to researchers funded by the National Institutes of Health.
Their study is the first comprehensive examination of brain structure changes in preschoolers with signs of ADHD, a disorder marked by a pattern of inattention, hyperactivity & impulsive behavior. The study appears in the Journal of the International Neuropsychological Society.
“Researchers were expecting to find the beginning signs of possible atypical brain development,” said James A. Griffin, Ph.D., deputy chief of the Child Development & Behavior Branch at NIH’s Eunice Kennedy Shriver National Institute of Child Health & Human Development, which supported the research. “What they found in their high-resolution scans was solid evidence that these structural changes already are prominent by age 4 or 5.”
Previous studies have documented brain differences in adolescents with ADHD. However, few studies have looked for such differences in preschoolers, despite research citing ADHD as the most commonly diagnosed psychological disorder among young children.
The research, led by E. Mark Mahone, Ph.D., was conducted at the Kennedy Krieger Institute in Baltimore. The study included 90 young children: 38 typically developing preschoolers and 52 preschoolers with symptoms of ADHD.
The children’s scans revealed that those with ADHD symptoms had multiple areas with less brain matter volume than their typical peers, & these differences were consistent with…

And if I'm will decide to get implants, which are medical devices, there are even more questions. Saline or silicone? Which style? How much monitoring is needed?
These are common questions people have when considering breast implants.
That’s why the U.S. Food & Drug Administration offers online tools to help people who are seeking information, as well as advice, when making breast decisions about breast implants.
Know the Basics
The FDA has approved implants for increasing breast size in women (augmentation), for reconstruction after breast cancer surgery or trauma, & to correct developmental defects. Implants are also approved to correct or improve the result of a previous surgery.
A number of studies have reported that a majority of breast augmentation & reconstruction patients are satisfied with the results of their surgery.
The FDA has approved two types of breast implants for sale in the United States: saline (salt water solution)-filled & silicone gel-filled. Both have a silicone outer shell and vary in size, shell thickness, & shape.

Islet Transplantation Improves Quality of Life for People with Hard-To-Control Type 1 Diabetes

News Release
Wednesday, March 21, 2018 NIH-funded study finds consistent, dramatic improvements among clinical trial participants. Quality of life for people with type 1 diabetes who had frequent severe hypoglycemia — a potentially fatal low blood glucose (blood sugar) level — improved consistently and dramatically following transplantation of insulin-producing pancreatic islets, according to findings published online March 21 in Diabetes Care. The results come from a Phase 3 clinical trial funded by the National Institute of Allergy and Infectious Diseases (NIAID) & the National Institute of Diabetes and Digestive & Kidney Diseases (NIDDK), both part of the National Institutes of Health.
The greatest improvements were seen in diabetes-related quality of life. Islet recipients also reported better overall health status after transplant, despite the need for lifelong treatment with immune-suppressing drugs to prevent transplant rejection. Researchers observed these improvements even among transplant recipients who still required insulin therapy to manage their diabetes.
The trial enrolled 48 people with type 1 diabetes who had hypoglycemia unawareness — an impaired ability to sense drops in blood glucose levels — & experienced frequent episodes of severe hypoglycemia despite receiving expert care.
Previously reported clinical outcomes from the trial showed that islet transplantation prevents severe hypoglycemia & improves blood glucose awareness and control. The study was conducted by the NIH-funded Clinical Islet Transplantation Consortium.
“Although insulin therapy is life-saving, type 1 diabetes remains an extremely challenging condition to manage,” said NIAID Director Anthony S. Fauci, M.D. “For people unable to safely control type 1 diabetes despite optimal medical management, islet transplantation…

The study found that during the first two years after high school graduation, 23 percent of young adults had ridden with a marijuana-impaired driver at least once, while 20 percent had ridden with an alcohol-impaired driver, and 6 percent had ridden with a driver impaired by glue or solvents or harder, illicit drugs, such as amphetamines, opioids, cocaine.
